Curriculum Proposal Form
Use to propose curriculum changes.
Supplemental Information Form – New Course
This form is for collecting administrative components of courses necessary for course creation. It must accompany all Category 1 new course proposals.
Curriculum Consultation Request Form
Use to consult with other departments, units, or faculties that may have an interest in or be affected by the proposed curriculum change. Originators complete the top half; respondents complete the bottom half.
Curriculum Consultation Report Form
This form is used to report on which units were consulted and to respond to concerns or issues raised during the consultation process.
Course Syllabus Template
Use to create a syllabus for a new course being submitted to Senate.
Library Consultation Form
Use to consult with the Library to determine what impact, if any, the proposed curriculum change will have on the Library’s services and resources.
Graduate Studies Curriculum Committee Report
This form is sent to the College of Graduate Studies for their consultation. Consultation with the College of Graduate Studies is required for all graduate-level programs / courses and if they will be affected in any way by a curriculum change.
Curriculum Budget Impact Form
Use to consult with the appropriate department/unit head, dean, and potentially the Provost (required for all new programs) to determine if additional budget will be required to implement the proposed curriculum change.
